Exercise-related transient abdominal pain, or ETAP, appears to stem from one of three mechanisms: breathing patterns irritating the phrenic nerve, reduced blood flow around the diaphragm, or postural misalignment during movement.

The phrenic nerve, which controls your diaphragm, becomes irritated when breathing rhythms fall out of sync with your stride. Some runners inhale and exhale at the same point in their gait cycle, creating repetitive stress on this critical nerve. Others experience cramping when blood diverts from abdominal muscles toward working legs, starving the diaphragm of oxygen. Poor posture compounds both issues by forcing your core to work inefficiently.

Relief during a run involves three proven tactics. Slow your pace to reset your breathing pattern and restore blood flow equilibrium. Stretch by raising both arms overhead while leaning toward the opposite side of the stitch, which elongates the affected muscles. Apply gentle pressure directly to the cramped area while breathing deeply and steadily.

Prevention starts before you lace your shoes. Practice mindful breathing during training runs, coordinating inhalation and exhalation with your stride in patterns that feel natural. Eat substantial meals two to three hours before running, and avoid large fluid intake immediately beforehand. Both can cause GI distress that exacerbates stitches.

Build your foundation through core strengthening exercises like planks and dead bugs. Strong abdominal muscles stabilize your torso and reduce compensatory tension in the diaphragm. Maintain consistent hydration throughout the day rather than chugging water before runs. Gradually increase your running intensity and duration rather than jumping into high-speed workouts when your body isn't conditioned.
